 Our dedication to patient satisfaction means setting you up for success before you arrive. Follow these helpful tips to prepare for your next Virginian Rehabilitation and Wellness visit:

  • Click Here to fill out our required new patient forms. Please have them prepared before arriving at your first appointment. If you have any questions, please call for clarification – we’re happy to help!

  • Keep your insurance card handy when calling to make your first appointment. Our friendly staff will request some basic information to verify your therapy benefits and provide an estimation of your financial responsibility before starting therapy on your first day.

  • Please bring your insurance card and driver’s license to your first appointment for our records.

  • Our physical therapists, DPT's, are direct access providers. However, a prescription ordering therapy may be needed if using insurance benefits. Please bring this with you to your first session or send it to us in advance (via fax, text, or email).

  • We are Medicare certified providers and are in-network with most insurances. Please call our office for more information. We also provide private pay options.

  • We recommend you wear loose-fitting, comfortable clothing to your physcial therapy or occupational therapy appointments.

  • We do our best to accommodate your schedule.
